A useful new tool

Visalia Medical Clinic is pleased to offer complete developmental screening for your child though the use of the  Child Health & Development System (CHADIS).

CHADIS is a child development assessment tool that helps you understand your child's developmental progress. Early identification and treatment of developmental problems imageis essential to promote optimal child development. CHADIS is also an excellent tool in the diagnosis of ADHD.

Please complete the CHADIS questionnaires prior to your child's next well visit or camp/school physical. Teachers may provide online input for your while with this tool, which is very helpful to us.

To use CHADIS, please go online to register at http://www.chadis.com/ and follow the initial registration prompts. On the home page, click New Users Register Here, and as a new user, you will be asked to enter the Invitation Code that links to our office. The Invitation Code is our office telephone number:

5597387506

Once you enter the site, you'll be asked to create a username and password for yourself.

If you have an appointment within the next two weeks, please login to CHADIS before your appointment. Your doctor has assigned questionnaires for your upcoming visit.

Thank you for your time and effort in completing CHADIS questionnaires. Your use of CHADIS helps us understand your child better. It also helps us guide you in the important task of enhancing your child's developmental progress.

Visalia Medical Clinic offers flu shots. The federal Centers for Disease Control recommends that everyone over the age of 6 months get a shot. There are certain people who are more strongly encouraged to get a shot, including seniors, those with chronic lung disease such as asthma or COPD and people with diabetes or kidney disease, to name a few.

Also known as pertussis, this is a highly contagious upper respiratory infection that can be very serious for infants. To make an appointment at Visalia Medical Clinic for the vaccine, call 739-2000.
